Attention: You can help us expedite the process of getting your brace covered by your insurance by downloading our prescription forms below and taking it to your doctor to sign. If you already have a signed prescription from your doctor for a brace or support and would like Southern Medical & Adaptive Solutions to bill your insurance please email your signed prescription to
